No matter where you work, the unexpected frequently occurs. There might be an accident, equipment breakdown, delivery delays, cost overrun and frequently personnel problems. Even in schools these incidents happen. On these occasions you need to write them up, you need to prepare an incident report. An effective incident report should cover the following points:

· What happened “ facts, think of the 5 W’s ( not opinionated)
· What caused it “ try a detailed chronological order
· What were the results “ injuries, losses, costs, damages
· What can be done to prevent a repetition “ evaluation

To make the report more effective it is good to use a graphic: Refer to Chapter 4 for this.
