Needs Statement

Write two paragraphs outlining the problem that is occurring within your geographic location. The name of my program is “Safe Haven for Homeless Pregnant Youths”. I am trying to apply for a grant for my program which will help continue to prevent and eliminate homelessness among pregnant youth and young adults.

Goals, Objectives, and Evaluation Criteria

          In this section, outline your goals, objectives, and evaluation criteria. I need you to write the two paragraph for the needs statements then complete the areas starting with the goals stating to create a sentence because I already filled in the blanks. Then complete everything starting with the objectives after that. I already had to do my goals before this is why I just filled it in for you.

These are my goals for my project:

Goal 1: Assist Youth and Young adults to Swiftly Move to Permanent or Non-Time-Limited Housing Options with Appropriate Services and Supports

Goal 2: Build Permanent Connections and Supports that Promote Self-Sufficiency and Social-Emotional Well-Being, including Educational and Employment Needs

Goal 3: Create a sustainable system to continue ending and preventing youth homelessness in Chatham County.
