She works at the casino . She came into treatment because her probation officer recommended it .
She is on probation for burglary . She does not remember the incident due to blacking out from alcohol She has used alcohol since the age of 12 on a regular basis and heroin cocaine since the age of 15 on a regular basis .She is diagnosed with F15-10 Amphetamine type substance disorder F-95 traumatic disorder, F 41.9 unspecified anxiety disorder and F32.9 unspecified anxiety disorder Alcohol use disorder severe. The client has been hospitalized 4 times in the last two years for alcohol induced seizures . The client is taking trazadone 150mg for sleep and vistril 50mg twice a day and Neurontin 300mg three times a day The patient has no allergies , the patient family history is her parents are both alcoholics and her sister died from an alcohol related death which was traumatic for her along with the loss of both of her kids , along with psychosocial stressors of problems with the legal system . She has her ged but no other further schooling. Has worked at the casino for the last 8 years. Treatment have the patient acknowledge powerlessness unmanagable caused by addiction, help patient see through cognitive behavior therapy relationship between addiction how substance abuse used to treat anxiety , develop a level of trust with the patient so the patient can talk about their issues of addiction trauma and anxiety . The idea is to get the patient to learn and implement positive coping skills that result in the reduction in anxiety and the abstinence of addiction. I would also use emdr for the treatment of trauma related to the loss of her sister and kids along with neurofeedback . I would help the client develop a structured program of recovery which would include getting a sponsor doing a 12 step program. Doing group and individual therapy 5 days a week for 60 days and continue and do after care for another 30 days.Learning Resources
