This is why she was referred to therapy:
Ms. Walker is the clients name after 10 years of marriage she went through a divorce. She was referred to therapy by her primary care physician for Major Depression Disorder. Ms. Walker is in the preparation stage of change where she is working towards taking action to manage depression. Ms. Walker described her mood as generally sad and indicated it has gotten worse in the past month since her husband requested a divorce. Ms. Walker has excessive fears and worries about lack of finances, unemployment, being a single mother and a divorcee. She denied hallucinations, delusions, obsessions or compulsions.
These are her Recommendations
1.) Ms. Walker will increase self-awareness by recognizing depressive thoughts/ feelings/ emotions verbally and resolve its causes over the next 180 days. 2.) She will eliminate feelings of hopelessness and helplessness by engaging in therapy, community support, and using her coping skills to redirect when triggered. 3.) She will decrease her anxiety by engaging in therapy, community support, and using her coping skills to redirect over the next 180 days. 4.) She will remain compliant with her medication(s) over the next 180 days. 5.) She will demonstrate an understanding of prescribed medication dosages and side effects and the rationale for their use.
This is the goal she has set for herself
I would like to discover the start of my depressive symptoms and learn how to cope with my negative thinking.
The case manager is now through with Ms. Walker sessions and she is referred to therapy. Case Manager concludes with her by stating: As we terminate our services, I encourage you to partake in The Rosenberg Self-Esteem Scale, which is used for evaluating individual self-esteem. I think this could be a good guide for you to learn how to measure both positive and negative feelings about the self. Ms. Walker agreed to explore the self-esteem and continue to journal. Journaling is what she was using as her daily homework to write down how she was coping with her stressors.
