When treating patients in primary care we will have to educate patients on maintain a health weight. The estimated percentage of overweight and obese people in american is at about 69%, low- income women are at the highest risk(Bickley, 2012). To calculate a patient Body mass index (BMI) we need their height and weight. When a patient is above 25 % they are overweight and above 30% they are obese (Bickley, 2012). When a patient becomes obese this can cause them to many health problems like DM2 and HTN (Bickley, 2012). The first thing to do is educating the patients on their obesity, how its affecting their health, and how they can loss the weight. The patient has to want to loss the weight otherwise this wont work. To help a patient to lose weight the have be educated on the food they eat, this means getting a nutritionist on board to show the patient what they should be eat and avoiding (Aboueid e al, 2018). The next can be getting a mental health involved is there something that causing the patient to overeat (Plourde et al, 2012). The patient also needs to exercise getting them help or educating them on how much activity the have to a day (Plourde et al, 2012). The barrier would be for low economic patient eating healthy would buying food that is healthy since food that are high in carbs is cheap and easy to make or get (Aboueid e al, 2018). The key would education obesity, trying to prevent it before it becomes a problem for the patient.

There are several studies done on teen pregnancy that cover those who are at a higher risk for teen pregnancy and whether race, economic status, and even if having a mom or older sibling with a teen pregnancy plays a role.

Findings- One article showed that the teens living in Alabama, Arkansas, Louisiana, Oklahoma and Texas have some of the highest teen pregnancy, births and STD’s in the United States (Jozkowski & Crawford, 2016).

Sex education-Part of the challenge for sex education in school is that there is not a standard curriculum that is suggested or mandated, each state and each school is left on their own to decide or not decide what should be taught (Jozkowski & Crawford, 2016). There are even some experts that have the opinion that sex is too big of topic and schools are not equipped to cover this important level of education (Palmer, 2016). There is a set of guidelines Sexuality Information and Education Council of the United States (SEICUS) that was established to cover some key topics including the teaching of anatomy and physiology, the difference between abusive and non-abusive relationships, STD’s, and safe sex (Palmer, 2016). This same article supports my opinion that this is a sensitive topic but it should still be taught. My personal recommendation is that if sex education is going to be taught in schools there needs to be a specific set of standards that are covered and that it should be taught by professionals. I believe that who teaches the class is just as important as what is taught.

Birth Control- the medical provider (us in this case) will play a key role in education, we can create the safe environment for both the parents and the teen. Taking the time to let both of them know that what is discussed is confidential, conversations can be done together and separately so that each can be offered confidentiality (Breuner & Mattson, 2016). While it is best to teach that abstinence is the best birth control and for preventing STD’s, there is a need to discuss the other options. If possible, it would be great if the teen was able to go to their parent with their questions and needs but if not then we need to be the trusted advisor they can come too (Gupta, 2017). There are so many methods to offer and what type of sexual activity are involved with, but for males condoms would be ideal and for females pills or shot would be ideal (Gupta, 2017).

There has been many research studies done on the topic, teen pregnancy, due to their high prevalence not only in the U.S. but in other countries as well. There has been systematic reviews, mixed-methods systematic review to look at ways to educate teenagers, how to prevent teenage pregnancies as well as how to deal with them as they arise.

Research Findings

Teenage pregnancy has been the highest among those that are lower income, eduation, social support networks, and living environment. It also has been studies that 4 out of 10 African-American females will be pregnant by their 20th birthday (Danawi, Bryant, & Hasbini, 2016). Teens in the United States are also more likely to become pregnant than those in other countries. Researchers believe that when targeting teenage pregnancy prevention that they should target those with inequalities rather than teenage pregnancy directly (Danawi, Bryant, & Hasbini, 2016). They also make a point that they believe teenage girls that think they will not attend college or have other limiting factors related to higher education will end up pregnant than those that have a planned education or college plans.

Sex Education in Schools

Sex education should not be addressed in a way to only promote using protection or being careful. Research has shown that teaching students where they can buy contraception is not a positive outcome to decrease teenage pregnancy (Danawi, Bryant, & Hasbini, 2016). Creating policies to reduce the the poverty and create more equal opportunities for those will have a greater impact on reducing teenage pregnancy. As it shows, poor living conditions or environment, low income, bad social networks has a greater influence on teens getting pregnant than anything else. Focusing on those problems might make a bigger difference in this problem in our country.

Another study recommends that teens should feel like they have a choice and that they are being listened to when it comes to being sexually active. As not all teens are going to stay abstinent, regardless of the education that is provided, these teens should know their options of birth control and support that is available. Providers should offer longer opening hours for those that have to attend school as well as location that is within a good location for them (Charles, Rycroft-Malone, Aslam, Hendry, Pasterfield, & Whitaker, 2016). Teens needs to know they can trust their teachers and primary care providers to get that information as well as support if they have questions or concerns. They will get this information from someone, so it would be better if they received it from a doctor, nurse, teacher, parent, etc.

Birth Control Talk for Parents

It is not always an easy topic for parents to discuss sex education, birth control, abstinence, or pregnancy with their teen. Teens are more than likely not wanting to willingly bring this topic up to their parents. Studies do show that teenagers with parental involvement are less likely to become pregnant as well as delay sexual initiation (Charles et al., 2016). No matter how comfortable or uncomfortable this subject may be, it needs to be addressed. Parents need to show their teens that they are listening and that they can be trusted to confide it. Avoiding the subject is not going to stop it from happening. Parents should be willing to discuss this with the child’s primary care provider as well if more advise is needed. Birth control can be something that the primary care provider can provide various information on and which one to choose if that is one of the options for that teen.

For this week’s discussion, we were asked to discuss strategies and methods that would help our patients reach their ideal weight. According to the Centers for Disease Control and Prevention (CDC; 2009), nearly 34 percent of American adults are obese, and an additional 34 percent are overweight. In nearly two-thirds of states, more than 25 percent of the adult population is obese. Obesity rates have contributed to worsening health outcomes and an explosion of health care costs, as overweight and obesity significantly increases the risk of developing more than 20 different diseases and health conditions, including Type2 diabetes, hypertension, metabolic syndrome, cardiovascular disease, specific cancers, and osteoarthritis (para 3).

As Family Nurse Practitioners we are typically the patient’s primary care provider (PCP), we are the patient’s primary point of care contact. Studies have shown and concluded that screening and counseling for obesity are not occurring regularly during primary care office visits (CDC; 2009). Furthermore, recent literature has indicated that nearly 50 percent of primary visits did not include a record of the height and weight data necessary to calculate BMI. Additionally, clinically obese patients (BMI ≥30), 70 percent did not receive a diagnosis of obesity and 63 percent did not receive counseling from their physician (CDC; 2009).

What I can do as a Family Nurse Practitioner is properly documenting my patients BMI with their height and weight as well as offering teaching and counseling about the health risks associated with obesity and how they can make better lifestyle choices. Modifiable factors would include, diet, physical exercise, and social activities.

A process of performance management is developed in companies to better

shape how employees execute their job responsibilities and complete their work.

Ideally, employees should feel comfortable with this process, believing that the

Jewelers Mutual Insurance Company (JMI) is one such company that has

actively improved its performance management approach, and the results have

been very encouraging. Employees were initially dissatisfied with the feedback

and goal-setting approaches that were being utilized to manage job performance,

so company leaders decided to involve employees in the redesign efforts to

create a more viable program that would be satisfactory for all the parties

involved. An outside consultant started the process by interviewing top leaders in

the company, and focus groups were used to solicit feedback from various other

members of the organization. By utilizing a more participative and inclusive

approach, the company was able to identify the problems with the current

performance management system and generate greater support for the proposed

changes that would ultimately fix these issues. This case illustrates how

important employee participation is in the effective management of human

resources, particularly when developing a viable performance management

system.

Several key changes were made to the performance management system

based on the feedback received from managers and employees. In particular,

inconsistencies in the administration of the performance management system,

problems with the rating techniques and forms, and various challenges linking

pay to performance were specifically targeted as part of the redesign effort. Such

reflection and self-assessment prompted a number of specific improvements to

management of job performance within the company. Evaluations are now based

on narratives, various metrics of accountability, and job goals. Further, feedback

is provided to employees on a quarterly basis, compensation is more strongly

linked to individual effort, and the performance management system functions in

concert with the other elements of human resource management. The changes

made to the performance management processes at JMI Company demonstrate

how human resource professionals can work with other staff members to create

a system that excites employees and, ultimately, yields greater job

performance.

It appears as though officers are reluctant to join the CVPD. LA has a high rate of crime and not enough staff to control the crime. The superintendent of the CVPD appointed a research team to find out why individuals were opposed to joining the CVPD. This is what the team found:

 

In an effort to restructure the police force, there is a sudden absence of well-defined roles. New members of the department are confused about the roles they are required to play. Most of the time, there is too much work pressure because they are required to perform functions beyond their expertise. Sometimes, there is no work at all. This causes low morale and low job satisfaction within the employees.

 

Another significant finding of the research done was that there is an abuse of power in the police force. Recent employees have reported that the sergeants tend to misuse the power they have. Not only do they expect the new employees to obey them blindly, but they also make unfair demands. For instance, three new police officers were put on night patrol duty for three months. During these three months, they were made to do 72 hour shifts on many occasions.

 

A very crucial finding was that the new employees who have joined the police force as part of the family tradition are disillusioned with the changes in policing. They are not prepared for the new attitude toward policing and the new policies that have developed.

 

For this assignment you are the recruitment officer in the Centervale Police Department (CVPD), you have been given the responsibility of helping CVPD solve the problem mentioned above. The empty positions in the department need to be filled as soon as possible. To do this, changes have to take place within the organization.
