The 1980s saw profound change within organisations and how they are managed. These changes took place around the World, and are still impacting upon your life today – the most obvious way being tertiary tuition fees. What you may not know is that these global changes were instigated here in New Zealand. Our beautiful country is still seen as the global laboratory for many economic and social changes.

 

As an introduction to the topic, you will need to watch some documentaries which provide a good insight into the changes of the 1980s. I suspect that what you will see – and subsequently read about – may well make your hair curl! Your parents and grandparents who lived through the implementation of the changes will be a very useful resource as you do this assignment.

 

 

Considering the structural and cultural changes taking place in Aotearoa New Zealand in the last decades, you are required to write a 2500 word essay which critiques the impact of these changes over organisations and the workforce.

 

It is important to remember that there is no right/wrong answer to this question. What is important is the way in which you support your arguments with evidence.

 

 

For this assessment, we will watch one of the following documentaries and discuss its relevance in class. Feel free to watch them in advance. There will be also additional readings available on Stream to support your arguments in the essay.

Chest Pain- SOAP

S: Mr. Tom Jones, is a 45 year old male, who has come to the office today with complaints of chest pain. He is a full -time air traffic controller for a large airport. He is married, and has 8 year old twin girls. His maternal grandfather had a heart attack at age 50. His family history is otherwise unremarkable. He describes the pain as chest tightness that does not radiate to his shoulder or jaw. He also states that sometimes he gets sweaty, rapid heart rate, with no shortness of breath. They symptoms occur frequently while at work, but if he takes a break it will lessen. He denies having the pain on his days off. He states that he is worried that he will have a heart attack just like his grandfather and leave his family behind.

O: Vital signs- HR 88, R 20, BP 140/85, T 98.9 Height- 6’ 1”, Weight 220lbs. The patients HR is within normal limits, an EKG show no ST elevations or other abnormal findings, his lungs were clear, normal reflexes in all 4 extremities.

A: This patient is most likely experiencing anxiety attacks from his high stress job. This would be why when he takes a break from work or his days off that the symptoms lessen or are not present.

P: Starting with reassurance for this patient, that most likely he is experiencing anxiety attacks that are related to his job combined with the added stress of leaving his family at an early age. It would be my recommendation that he see a cardiologist to establish a baseline stress test given his family history of heart attack at a relatively young age. For the anxiety attacks, he could consider a less stressful job, if this is not an option, learning coping skills such as regular breaks to walk around and relax. Prescribing anti-anxiety medications: Xanax, Klonipin, Ativan or Valium. Follow-up should be within in one month to re-evaluate the medications and their effects.

Questions to ask the patient

What are you doing when the pain occurs? Asking this question will determine if the pain is sudden when they are at rest, during exercise or during a stressful situation.

Does anything alleviate or make the pain worse? This allows us to see what the patient does to help the pain, or does he just endure without trying to help it go away.

On a scale of 1-10, 10 being the worst how would you rate your pain? This asks is the pain the same each time or does it change from time to time.

Where is the pain? Location of the pain is significant to defining if it is a cardiac event or not.

Do you have shortness of breath? Generally, anxiety attacks do not include shortness of breath, unless there is added panic related to the pain.

How would you describe the pain (stabbing, throbbing, crushing, sharp, etc). Radiating and crushing pain is more specific to a cardiac event.

Findings

Normal: It would be normal if the pain was related to stress/anxiety that when the patient is removed from the situation that the pain would get better. It would also be normal if the patient hasa panic feeling with the pain from the fear of leaving his family.

Abnormal: If the pain did not go away when stepping away from a potential situation. Abnormal if the pain happened if the patient was at rest when the pain came.

S (Subjective)

A 67 year old female with history of hypertension, hyperlipidemia, hysterectomy, and CHF with last ejection fraction of 55% presents today with chest pain. Patient complains of pain that “feels like someone is squeezing my chest as tight as they can” with pain on the left side of the chest and radiating to the back. Pain is 8/10 pain. Patient is nauseated, but denies vomiting. Per patient, the pain is worse upon exertion and gets better while resting. Chest pain began this morning when she was watering her plants. Patient ignored it until it worsened and she felt short of breath and the pain “really worried me”. Patient complaints of a cough that started last week and has been spitting up green phlemn.

O (Objective)

Vital signs are as follows: T: 37.0, BP 165/98, HR 98, R 26, SaO2 93% on 2L NC. Patient’s skin is pale, diaphoretic. Patient appears to have moments of distress and holds chest while grimacing during conversation.

A (Assessment)

Neurological: patient is alert and orient to person, place, and time. Moves all extremities strong and to command. PERRLA size 3mm. Speech clear. Patient denies headache, numbness, tingling, double or blurred vision.

Cardiac: Heart murmur heard upon auscultation. Heart rhythm regular. 2+ Pitting edema noted in lower extremities from ankles to knees.

Respiratory: Crackles heard upon auscultation throughout posterior chest. Respirations mildly labored. Chest symmetrical.

GI/GU: Bowel sounds present. Patient denies abdominal tenderness. Patient reports last bowel movement this morning. Patient urinating regularly, but states “not that much lately”. Patient currently NPO.

P (Plan)

Current plan at this time is to get 12 lead EKG. Basic Metabolic panel and complete blood count, troponin, CK, lytes, BUN, creatanine, BNP. Will repeat troponin every 6 hours times 3. Keep patient on oxygen. Will prescribe 2mg of morphine for chest pain to give now. Nitroglycerin ordered as needed for chest pain to give every 5 minutes x3. Monitor patient on continuous cardiac monitor to watch for any EKG changes. Chest x-ray. Will plan to give lasix for crackles and edema noted after chest x-ray is resulted. Place foley catheter to monitor strict intake and output.

Questions

First I would ask the patient if the coughing causes any discomfort or worsens chest pain. Many respiratory symtpoms such as coughing can mimic chest pain thought to be cardiac related. Respiratory disorders such as pnemonia or pulmonary edema may cause chest pain symptoms, but are not cardiac related (Bickley, 2017).

A second question I would ask the patient is about her normal physical activity and tolerance to activity. The patient stated that she was outside watering the flowers, but it would be important to know if this is something she does often or if it was the first time she has been doing physical activity. This would allow me to understand if the shortness of breath came from over exersion or if it was only felt after the chest pain began.

Normal findings: patient stated chest pain worsened when coughing. Chest pain subsided when resting.

Abnormal findings: patient stated she was out of breath as soon as she walked outside and has been unable to perform normal activities as she was doing a few weeks prior without being winded. Patient has been coughing up green phlegm for a week.
