Interactional theory is based on the premise that leadership behavior is generally determined by the relationship between the leader’s personality and the specific situation.

Systems Theory and the Difference Between Open and Closed Systems

Closed system models consider the external environment to be stable and predictable, and they assume that it does not intervene in or cause problems for the functioning of an organization, therefore closed system models do not depend on the external environment, they are enclosed and sealed off from the outside world. An open system within the organization depends on its supporting environment for continued inputs.

Description of Problem Identified in Your Current Organization

Leadership effectiveness requires the ability to use the problem-solving process: and maintain group effectiveness, communicate well, demonstrate leader fairness, competence, dependability, and creativity. In my current organization we recently had a staffing crisis, staffing retention posed a huge problem. Changes are needed in how the unit is managed and organized, the orientation process needs to be completely changed and reorganized. Instead of problems solving and coming up with a new way to retain staff, more work was put on to the current nursing staff to cover the shortage, which in return seasoned staff began to get burned out and quite therefore adding to the already staffing crisis.

Explain the Problem from a Closed-System Perspective

The current situation was currently dealt with from a closed system perspective, the management staff did not look to the organization itself to help with the issue, all of the issues were dealt with internally, the nursing float pool from the main hospital campus was not utilized, a new orientation model was not implemented, so this still remains a huge issue in the retaining of the new staff.

Describe how the Problem Could be Addressed by open-system perspective

In an open system the organization adapts its functioning in response to negative feedback and external information signals through series of iterative adjustments that will allow the system to evolve. The negative feedback that has been given to the management staff should be used as basis of problem solving, looking at other orientation models and concepts from other areas that are similar to ours, and simply by showing staff and getting the staff involved in the changes would help to show that they are trying to improve the unit.

Literature Searches

An important step in the EBP process is reviewing the current body of literature to better understand the subject or topic in which you are interested. By conducting a review of the literature, you are building foundational knowledge about the topic; later, you can use this background to build new insights. Developing a strong grasp of a topic can only be garnered by taking the time to thoroughly search for relevant information and resources.

For this Discussion, you will practice searching the literature to find evidence on a specific topic.

To prepare:

Choose a simple search term(s) relating to a topic of your PICOT question.

Review the information on the evidence hierarchy discussed in Chapter 2 of the course text, in the article, “Facilitating Access to Pre-Processed Research Evidence in Public Health,” and in the multimedia presentation “Hierarchy of Evidence Pyramid,” found in this week’s Learning Resources.

Review the information on the Walden Library’s website, “Levels of evidence.” Take a few minutes to explore the different types of databases available for each level of evidence and focus on the meaning of filtered and non-filtered resources.

Summary of Search

The topic I want to research is the value of 12 hour shifts on patient safety. I am interested in finding out how longer shifts on nurses affect the safety of patients. The search term I used was “12-hour shifts” and “patient safety”.

While searching this topic in the Cochrane Database, I entered the search terms which provided 0 results. I changed up my terms to “nurse staffing” and “patient safety” and that yielded one article.

While searching for unfiltered information, I used the CINAHL and Medline databases with the same terms, “12-hour shifts” and “patient safety”. This search yielded a total of 65 articles.

In terms of background information, I did a search on my own to find out the views of the American Nurses Association (ANA). The ANA (2014) has offered up a position statement encouraging nurses and employers to consider work assignments to reduce risks and create a safe culture.

Types of Information

The filtered resources found in the Cochrane Database contains systematic reviews, critically-appraised topics, and critically-appraised individual articles (Walden University Library, 2012). Polit and Beck (2017) describe systematic reviews as a pivotal aspect of EBP with a “bottom line” summary of the best evidence to use.

The unfiltered resources found in the CINAHL and Medline databases yielded the most articles. Randomized controlled trials, cohort studies, and case-controlled studies are found in this kind of search (Walden University Library, 2012). Polit and Beck (2017) confirm that randomized controlled trials (RCTs) are effective for researching healthcare interventions.

Background information and expert opinions can come from several different resources such as textbooks and conferences (Walden University Library, 2012). These sources are not always backed by research studies.

Useful Tips

Some tips I found useful were to be flexible with the search terms. If you do not yield a good batch of articles, consider changing up your phrases. Make use of the connectors “AND” and “OR”.

PICO Question

In the forensic psychiatry patient population, how does the “forensic” label affect the therapeutic relationship, care, and compassion from nurses in comparison to the patient who is non-forensic?

Research Results

Following are my results in researching this topic using the levels of evidence pyramid found on the Walden library website. The first three levels use filtered resources, which are “resources that appraise the quality of studies and often make recommendations for practice” (Walden University Library, 2012, table 3).

For the level one research, systematic reviews, I chose the Cochrane Database of Systematic Reviews and my search topic was forensic psychiatric nursing. Only one article popped up and it was totally irrelevant to my PICO question.

For level two, critically appraised topics, I searched the National Guideline Clearinghouse with the same search topic. This time four articles came up but only one was relevant to my topic.

For level three, critically-appraised individual articles, I used the Evidence-Based Nursing search engine using the same topic and was presented with three articles, and again, only one was relevant.

An un-filtered search, you are searching primary, unfiltered literature, that the researcher needs to review for validity and reliability (Walden University Library, 2012).

In searching for a randomized control trial article, I used the CINAHL Plus with full-text database. I narrowed the search to randomized control trials only and used the topic psychiatric forensic nursing. Only one article popped up and was totally unrelated to my topic.

For a cohort study, I used Medline with full text. This time for my search I typed in “forensic psychiatric nursing and cohort studies” and expanded the search to include any of the terms within the whole text. This one got great results with 365,023 articles found.

Finally, for case-controlled studies, I again used Medline with the same expanders. This time I entered “forensic psychiatric nursing and case studies and case-controlled studies”. I received 3,737,341 articles.

In comparing the articles, I selected, I found the articles from the top three levels to be more detailed and informative, however, there were very little articles to choose from. These articles appear to be more valid and at the end, they offered ways to implement or recommended changes to practice. The bottom levels gave me many more articles to choose from, but with each, it took some time to make sure the article was valid. “In some scales, Level 1 evidence is the strongest evidence and in others, Level 5 is the strongest. Some levels of evidence scales are based only on the research design and others provide levels identified by the judgment of the quality of the study itself” (Thompson, 2017, para. 11).

I would recommend putting expanders in the search to include full text and any keyword from the search phrase. “Keywords are the search terms that you use to describe the topic you want to retrieve. The database will word-match your keywords against the text of the article, and deliver results that match what you enter” (Walden University Library, 2018, para. 1). Once I did this, I received thousands of articles to choose from.

Formulating a question that targets the goal of your research is a challenging but essential task. To prepare:

Review the article, “Formulating the Evidence Based Practice Question: A Review of the Frameworks,” found in the Learning Resources for this week. Focus on the PICOT model for guiding the development of research questions.

Review the section beginning on page 71 of the course text, titled, “Developing and Refining Research Problems” in the course text, which focuses on analyzing the feasibility of a research problem.

Reflect on an issue or problem that you have noticed in your nursing practice. Consider the significance of this issue or problem.

Generate at least five questions that relate to the issue which you have identified. Use the criteria in your course text to select one question that would be most appropriate in terms of significance, feasibility, and interest. Be prepared to explain your rationale.

Formulate a preliminary PICO question—one that is answerable—based on your analysis. What are the PICO variables (patient/population, intervention/issue, comparison, and outcome) for this question?

It is recommended that you use Microsoft Visio to create the models although you may if you prefer use Word, PowerPoint or appropriate alternatives.

Part B: Open Source Software Comparison Table

In this section, you should conduct research into a suitable software solution for CC Music. You should decide on the set of characteristics which you will use to evaluate the software and your research should consider 4-5 alternatives in detail. This section should be presented as a table.

Part C: Report

In this section, you should write a report which provides an overview of the current situation for CC Music together with a roadmap outlining how the proposed changes to the business can be achieved to the benefit of the business. This should draw on your analysis in Part A, include your recommendation for software in Part B, and provide recommendations for ensuring that the strategy is effectively implemented, including consideration of the challenges ahead.

This section, should follow standard report structure:

Part D:  Reflection on your contribution to the online discussion and wiki

In this section you should submit a 350 word reflective summary, accompanied by your own self-assessment of your contribution to the online elements throughout the module, using the table labelled Reflection Self-Assessment Pro forma in Appendix A (copy and paste it into your assignment document). The reflective report should include your detailed reflection, supported with evidence (pasted URLs) from the online discussion and wiki, detailed below:
