ASSESSMENT OF THE RESEARCH PAPER ON AN ENVIRONMENTAL ISSUE

This assignment is a research paper on an issue related to the human impact on the living environment, habitats, land use and natural resources. You must bring to the analysis of the selected topic (you have to choose a topic from the list below) a thorough feminist critique that elaborates on the chosen subject and integrates concepts of gender, race, class, and social justice. Be aware that a critical analysis is essential for this assignment and that critical thinking does not imply negativity but shows that you evaluate the texts you read academically. The research paper should be in essay format but answer all six questions (see A-F below) by integrating the answers rather than answering them one-by-one to enhance the flow of the text. The paper topics* are:

  1. Biodiversity (loss & conservation of biological diversity);
  2. Climate change (global warming, greenhouse effect, climate justice, etc.);
  3. Deforestation (illegal logging, rapid pace of destruction, etc.);
  4. Extraction of natural energy sources (either A: Tar sands mining; or B: Fracking);
  5. Waste (landfills, incineration, various types of waste produced from human endeavors, etc.)

*Be aware that only papers on one of the topics from above are accepted; if in doubt about your topic, contact me. Use the following questions as guideline and try to answer them as appropriate:

Think globally! Explain in your paper where the environmental issue you have chosen is most prominent – outline the importance of tackling the chosen issue at global, national, and local scales if possible.

Your work is required to have a length between 1400 and 2000 words and include a title page and reference list (title page, reference list or possible appendices are not included in word count).

At least two suitable articles from peerreviewed/scholarly journals and/or non-fiction books should be incorporated (use APA format for in-text citations and for the reference list).

Non-scholarly material from the courseware package and the internet can be used in addition to the two peerreviewed/scholarly references (provide the page reference to the original document for direct quotes if possible, not the page number of the courseware pack).

All pages (and figures and tables, if incorporated) must be numbered; all citations (and figures if included) must reference its original source (use APA!).

This assignment (WORD document) has to be submitted electronically only via Blackboard before 3:30 p.m. on March 13, 2018. I currently work at Memorial Hospital in Gulfport, MS. Memorial is a not-for profit hospital with 303 beds and 70 clinics. According to Memorials mission and vision, Memorial is “governed by a Board of Trustees, strategic decisions are based on six core strategies: Customer Satisfaction, Growth, Work Environment, Finance, Quality/Patient safety/information Technology (IT) and strong hospital/physician relationship” (Memorial Hospital, 2017). I have been a nurse for 19 years but have been in a leadership role for the last 3 years. Attending meetings daily with hospital administrators is something that is new for me. Being involved in decisions that affect the entire culture of the facility can be overwhelming. “The culture of an organization is how they do things” (Watkins, 2013). I have found that customer satisfaction is a big part of Memorial’s vision and it is a high priority for administrators. Growth is also something that is a high priority. The administrators are frequently buying clinics in our surrounding area to offer more services to patients in a broader area. I have also noticed that finance is a huge topic lately. Because we are not-for profit and depend on government reimbursement, changes are frequently made for nursing staff so that reimbursement is higher. The climate of our organization lately has not been positive. Most of the staff at our organization have been there for many years and have seen years with no merit increase and shift differentials being taken away. At the same time, we have also seen administrators buying 10-15 clinics in surrounding areas. It’s difficult for staff to see money spent on buying clinics when we do not get a merit increase for years. The staff has a very negative attitude when asked to go above and beyond in the daily duties when they are not receiving anything in return. We have lost 30 employees in one year in my department due to lack of merit increases and due to other hospitals in the area offering to pay more per hour. Due to the negative climate in our organization, it would be beneficial for administrators to change the culture of our organization.

Foundations of an Organization and Organizational Assessment

The goals, behaviors and what holds importance for an organization can be summed up in the mission, vision and values statement. The statements made by an organization are not always a true representation of what goes on. To facilitate an environment of positive behaviors among all staff, it is crucial that leaders mirror the presentation in which the organization aspires to be. (Nelson & Gardent, 2011)

Mission, Vision, and Value Statements of Personal Organization

The mission and vision statements speak of exceptional care, respect and becoming the preferred choice of service. The values statement is lengthy and involves focus on safety, trust, honesty, confidentiality, compassion, fiscal responsibility, and teamwork. Most of what is in these statements is supported, but too often, resources are wasted, and patient safety does not always appear to be the priority. Management often assists at bedside when needed, makes routine staffing and patient rounds to “check in”. Frequent unit and organization meetings are held to keep staff up to date with changes, successes and areas in need for improvement.

Culture and Climate of Personal Organization

Organizational culture are those definite standards set forth for the members and climate is how those members view the organization. Teamwork, support, trust and loyalty are those qualities consistent with the present culture. While these standards may be true for individual departments, it may not be as strong between units, resulting in gaps in communication and patient care. Climate of an organization is subjective and can fluctuate between job roles, departments and other variables present. (Marquis & Huston, 2017)

Role of the Nurse Leader

As a nurse leader, fostering a culture of respect and teamwork is important to the survival of the organization and patient safety. A positive organizational culture and climate can facilitate implementation and adoption of better practices, resulting in better patient outcomes. (Rai, 2013) Conclusion

Upholding the standards named by an organization by all members is vital to existence. Extenuation circumstances may impose an occasional moment where a re-evaluation needs considered but for all purposes, these are qualities that should remain intact. Creating an environment of trust and respect is essential to maintaining and enhancing an organizations mission, visions and values.

Deciding to end life-saving measures can be very difficult for the “team” and also the family. Families often think that the resuscitation team did not work long enough for the patient or did not do “everything” possible to save the patients life. The emergency room I work at is trying to involve family presence at the bedside during resuscitation, this way the family sees the effort the team is putting into the patient, they can see how brutal CPR can be. On the negative side, healthcare members were concerned that involving the family would cause the “family to be traumatized, the potential for the family to disrupt the resuscitating efforts” (Twibell, Siela, Neal, Waters & Riwitis, n.d). The benefits of family presence at the bedside is it “meet the needs of patients, families, and staff by providing consistent, safe, and caring practices for all involved in the resuscitation process” (Madden & Condon, 2007).

When using EBP practice to implement a new policy in the hospital, the decisions are “best made among a team or nurses working together to solve a common clinical problem” (Pilot & Beck, 2017 Pg. 38). My PICOT question would be “In the emergency room is the presence of family appropriate during an active resuscitation helpful with coping?” A barrier to the PICOT would be having family members or staff members that are highly against family presence. Another barrier that I think could come up is allowing the family to be involved, is this a violation of patient rights? I think there is an excellent opportunity for research to be conducted on this subject.

Patient falls are significant health care setbacks that often negatively impact patient health care outcome on my unit. One of the unit-wide interventions to prevent patient fall is having patients sign a “Fall Prevention Agreement” on admission. The agreement is patient’s commitment to call for help at all times and prevent existing bed unassisted. There are other fall prevention interventions that are used hospital wide, but this Fall Prevention Agreement signing practice makes me wonder the merit of the practice and its effectiveness. Inarguably, in an Intermediate care unit (IMU) that often sees patients with neurological impairments, cardiovascular impairments and pulmonary issues also have risks for poor comprehension of information. Besides, cognitive altering medications such as sedatives, sleep aids and pain medications impact patients’ understanding of their health situations.

Along these lines, I find no Evidence Based Practice (EBP) that supports signing a Fall Prevention Agreement would ensure that patients would always call for help prior to exiting their bed. Rather than signing Fall Prevention Agreement, I think fall prevention screening for EBP and identifying reliable interventions would help prevent patients’ falls risks. Pilot & Beck (2017) endorse health care practitioners question the effectiveness of clinical practices. Consequently, “Does a stroke patient on Neuro IMU signing fall prevention agreement reduce fall risk compared to fall screening over the hospital stay?”

While PICOT questions afford an opportunity for clinical research, Shivnan (2011) determines it is also an opportunity for the nurse the manager to create an action plan for staff to use EBP that evaluates fall prevention practices. Nonetheless, a potential barrier to this PICOT question is the manager’s acceptance of the need for fall agreement signage practice effectiveness as she is the innovator of the practice.
